Not usually - it doesn't generally require a local anaesthetic for example. It's probably more psychologically distressing contemplating a foreign body (ie a stitch) compromising one's bodily integrity rather than being particularly painful - though people nowadays do not seem to have any problems with cosmetic piercings.
