diagnostic questionnaire

Good evening.
Filling the questionnaire, what have I to keep in consideration, the current situation or the worst period (i.e. when I used to wake up once, twice three times per night feeling huge pain....)?

For most of it - the parts mainly related to the type of symptoms you have, it should be answered in respect of the symptoms which caused you to think you had a problem, even if they have now gone away again. However for the final part - which is preceded by an explanatory page about the source of the Boston-Levine questionnaire - those multiple choice questions should be answered in respect of an average day in the last two weeks.
