Questionnaire issues

Hi,
I first completed the questionnaire 9-8-18
Due to a now constant pain in my right hand, my GP has requested I complete the questionnaire again, print to paper and to give her a copy so that she could then email a referral.
I revisited the questionnaire today and answered the many questions within. A NEW severity score was produced as a result. However when I come to print the completed 'new' questionnaire, I find that it is the 9-8-18 questionnaire offered for printing (albeit with todays date at the top) with the scores calculated at that time. In MyCTS however I can see both my old and new scores.
How can I print my latest completed questionnaire for my GP to act on ?
The old one is actually fine, we already know you have CTS and you don't even need your GP - you can just refer yourself back to us to get it seen to again. I'll arange an appointment - I presume your contact details haven't changed? JB
