Worsening symptoms

Due to worsening symptoms, I completed another assessment on 14 November as instructed by the hospital. Unfortunately I wasn't informed that I needed to email you. Therefore am now doing so. If you need anything further, let me know.
We can see you today (Monday 30th November) as you are a well known patient from our point of view, if you can make it in to see us - about 2:00 pm would probably be ideal. JB
