Questionnaire completed 232030

Hello I have had nerve conduction tests done at Benenden Hospital 2 days ago and had a verbal report of severe CTS on my R hand and mild on L hand. I have now been advised by an NHS hand therapist of this excellent pathway and have completed the questionnaire. My ID number is 232030 Marian B.
I believe I have replied by email. If I am to take you on as a patient I will have to undertake my own assessment of your CTS I'm afraid so the trip to Benenden will have been largely wasted - sorry! JB
