Hi dr j bland I have been on this forum before as mikes. I have an appointment with you on 9th sept. I am not sure how to answer the 19 questions regards both hands and pain in last 2 weeks Mr a smith fused my left wrist on 31th July .I contacted you regards pain numbness in Right hand after steroid a injection ,i have been on your forum as MIKES and this might jog Your memory. I was taking pain relief for 3weeks after my op , now I just have a numb middle finger on my Right hand so maybe the steroid or medication I have taken has relieved the problems I had after the steroid injection I had.before this injection I was awake every night with pain. So how do I answer the questions if I have not the symptoms as before the injections

There was no questionnaire data attached to the old MIKES user ID so I've deleted that one. Your old postings in the forum should still show up but as 'anonymous'. That leaves you with just one registration on the site with your Canterbury ID number linked to it. The severity questions should be answered for symptoms in the last two weeks, regardless of what they might have been like previously and even if there are no symptoms - in which case all the answers will be '1'. I'll move this message thread to the Canterbury patients forum once you have had a chance to see this reply. See you on the 9th. JB

I have just completed the cts questionnaire and have a score of 2.9 and 13% in my right hand which would indicate I do not have cts. Over 10 years ago I was offered an operation for cts but declined .January I had a steroid injection as I could not sleep through the numbness and pain I returned in June for another injection as the pain and sleepless nights had returned
And the moment I just have numbness in my middle finger which could be due to the problem I have with the second injection . So when completing the form asking how the pain was in the last 2 weeks it is not a true reflection , as before the injection and when it wears off the pain will keep me awake at night ,the pain will go upto my elbow and I pain stays with me for a good hour after I get out of bed and I can not feel my hand or shake the pain or numbness away

It probably is CTS, we know you had a moderately bad right CTS in 2005. The questionnaire was not constructed to diagnose people who already had a firm diagnosis, especially when they had been treated. The subjective severity scores however are a valid indicator of how bad the symptoms feel to you now and can be used to measure how your symptoms change in response to treatment. JB

First I would like to wish you and your team a happy new year.
I had a appointment with you on 9/9/13 for nerve test and ultrasound test which showed that my doctor had injected cortisone into my nerve , you informed my this May cause permanent damage.
On 13/12/13 I had c.t. Surgery at Q.e.q.m margate .today I visited my G.P to check the scar,I pointed out that my middle finger is still numb and my index and middle fingers are both painful to bend as pre operation .
On both fingers the joints have swollen and my doctor informs me it is arthritis.
I am asking has the fact that cortisone has blocked my nerve my operation might not be successful or I am impatient and recovery takes time .

Your NCS showed grade 5 right CTS in September and nerve damage of this severity tends to be slower to recover. It may well be 1-2 years after surgery before it reaches a final end state. Visible swelling of the finger joints is not a feature of CTS so your GP may well be right about arthritic change there.

Regarding the possible intraneural injection there is hardly any evidence in the literature to tell us what the long-term effects are likely to be as there are only a handful of anecdotal case reports of such events. Some of the patients made a good recovery, some did not. Remember that we are not sure whether the injection was actually given into the nerve - it just sounded like a fair probability from the history of the event and the ultrasound findings. I was hoping for a copy of your operation notes from hand surgery but I haven't received anything yet. JB
