Self Referral info please

Hello,
My surgery’s physio confirmed CTS, and it is in my family. I’m in extreme pain and being right handed really struggling. My GP sent me this link for self referral as splint, exercise and pain relief not helping but I can’t see any route to surgery. Please cam someone advise where I should be heading?
